Even if your plan is not listed above, in many cases we can assist you with filing the claim if you provide us with your plan information. If you do not see your insurance in the list, please call our office.
For Tricare Prime patients we require a referral before scheduling an appointment. If you have a physical copy of the referral, please bring that with you to the appointment otherwise, appointment will be out of pocket
If you need to cancel or reschedule your appointment, we require 24-48-hour notice to cancel or reschedule appointment.
